Log in with your user name and password to fill in a Patient History Form. You must include digital clinical photographs for an accurate diagnosis. An overview picture and close up are required. To ensure privacy, all patient information submitted via the form will be encrypted. If you have all your information in a PDF or Word File you can simply send it and your images by clicking Submit History File. You will need to provide your preferred pharmacy's fax number and email address for any scripts.
You will then be issued with a unique case number and an appointment date. On your appointment date, your case will be reviewed, diagnosed and you will receive an email response. Please refer to your SC case number when making any enquiries.
If you have chosen to have a phone or video consultation you will be phoned to organise a time on that day for your consultation with the appropriate video link.
I cannot diagnose pigmented lesions accurately over the internet unless they are accompanied by clear dermatoscopic images.
If you have the patient's information in a Word or PDF file just Click the Submit History File tab opposite to present this information rather than filling in the text boxes.
